Spindale Family Practice strives to provide individualized, compassionate care for our patients through comprehensive preventive care; aggressive treatment of developing or existing conditions with the ultimate goal in providing quality aging. By empowering our patients through ongoing education, we encourage them to take control of their health and be active participants in managing their care. We view the relationship between a patient and healthcare provider as a partnership- by working together we achieve the best possible wellness outcomes for you and your family. Our approach is that of a “Patient Centered Medical Home” where you are our main focus and every patient is known to the providers and staff.
A Patient-Centered Medical Home (PCMH) is a team-based model of care led by a personal medical provider who provides continuous and coordinated care throughout a patient's lifetime to maximize health outcomes. The PCMH practice is responsible for providing for all of a patient’s health care needs or appropriately arranging care with other qualified professionals. This includes the provision of preventive services, treatment of acute and chronic illness, and assistance with end-of-life issues. It is a model of practice in which a team of health professionals, coordinated by a personal physician, works collaboratively to provide high levels of care, access and communication, care coordination and integration, and care quality and safety.
